This is a charge imposed every year for the use of the credit card. This allows the bank to make some income in a tight economy, and to recover the savings from the interest rate. Even if you are using the card purely for the balance transfer, and not for purchases, the account is still subject to annual fees, if applicable. Some banks do not charge an annual fee for the first year when you apply for a balance transfer credit card.
